Program Overview

The Self-Monitoring Blood Pressure Program is designed to empower our community members to take an active role in managing their blood pressure. The program provides educational resources and ongoing support. Participants in this program will be encouraged to monitor their blood pressure regularly, with a final report being submitted to their primary healthcare provider. Our goal is to reduce the risk of hypertension-related complications and improve overall health outcomes in our community.
