We’re excited to share our vision for a healthier, stronger North Central District through the 2025–2027 Community Health Improvement Plan (CHIP). Built on input from people across our nine counties, this plan focuses on what matters most to our community’s well-being.
1. Helping People Live Healthier Lives
Small changes can make a big difference—like moving more, sleeping better, and eating well. We’re supporting healthier habits by promoting walking programs, senior-friendly physical activities, nutrition classes, and sleep education to help everyone live well, every day.
2. Supporting Economic Stability
Good health starts with having a safe place to live and enough food to eat. That’s why we’re working to reduce food insecurity and housing stress, while connecting people to budgeting tools, food programs like SNAP and WIC, and other community resources.
3. Making Care Easier to Access
Getting to a doctor, finding childcare, or accessing basic services shouldn’t be a barrier to health. We’re exploring ways to expand local transportation, grow mobile clinics, and create more viable childcare options for working families.
Let’s Make It Happen—Together
This plan isn’t just a checklist—it’s a community effort. Whether you’re a neighbor, healthcare provider, educator, or advocate, your voice and actions matter.
